Goals

The goals of the pediatrics rotation are that the resident learns to recognize pediatric problems that can be managed by the family physician as well as to recognize those that require a referral to a consultant.

Objectives

Patient Care and Medical Knowledge

Residents will:

  1. Develop skills in performing age-appropriate histories and physicals on pediatric patients ranging from preemies to older adolescents
  2. Evaluate and then initiate therapy in pediatric patients with pneumonia, meningitis and sepsis in the newborn, 2 month- 2 year-old and the older pediatric patient
  3. Discuss the differential diagnosis, work-up and treatment of jaundice in the newborn
  4. Discuss the differential diagnosis, work-up and treatment of acute gastrointestinal enteritis in pediatrics
  5. Evaluate and treat hospitalized asthmatic pediatric patients
  6. Perform pediatric lumbar punctures, IV line placement and phlebotomy and documenting procedures
  7. Recognize and manage behavioral, medical and surgical problems of children and adolescents
  8. Be able to effectively interview patients of different ages up to age 18, as well as interact effectively with the parents of acutely or chronically ill children (including hostile or acutely stressed parents)
  9. Develop skills needed to assess when consultation with specialists (i.e., social workers, psychiatrists, pediatric medical specialists), as well as when referral to a tertiary care center is warranted

Practice-Based Learning and Improvement

  1. Be able to use the internet to access current literature about pediatric medicine
  2. Be able to critically evaluate literature about current pediatric care
  3. Be able to work well in the pediatrics wards under the supervision of community pediatricians
  4. Be able to work well in the private office of a local pediatrician
  5. Be able to implement changes in patient care based on new information obtained from Attending Rounds, clinical experience, review of the literature, office experience and other sources

Interpersonal Communication Skills

  1. Communicate effectively with patients
    1. Be able to communicate in a manner understandable to the patient and the family of a pediatric regarding any underlying disease, acute medical issue, anticipatory guidance, treatment plan and prognosis
    2. Be able to listen to and address the concerns of patients and their families
  2. Communicate effectively with colleagues
    1. Be able to work efficiently in the context of Pediatrics Team in a community hospital
    2. Be able to clearly communicate patient sign-out, both when giving and receiving sign-out
    3. Be able to work efficiently with the support staff in the care of the pediatric patient
    4. Be able to function in the office of a community pediatrician

  3. Assist patients and their families with accurate anticipatory guidance

*** Contact the previous pediatrics intern (see Resident Master Schedule) the day before this rotation to receive signout on your patients and to confirm rounding time.

Day Hours

Wed. - Fri. ~6:30 AM Finish pre-rounding on in-patients and meet with Dr. Sachs or Woo
    After rounds - work rounds, discharges, etc.
  1:00 PM Go to either continuity clinic or Dr. Sachs office
    ***Return to SMH if any admissions
  7:00 PM Return to SMH to sign-out to IPS Junior on call
Sat. 7:00 AM-7:00 PM Round on SMH in-patients
Sun. 7:00 AM-7:00 AM Sign-out to Peds night float after completion of work
Mon.   Post Call
Tue.   Day off

Continuity Clinic:4 clinics / rotation

Call Schedule:This is a no call rotation.

Educational Half Day:Attendance is required. You will be excused for clinical duties.

Mon.-Sat. 7:00 PM-7:00 AM Round on SMH in-patients

Sign-out to Peds Day float after completion of work
Sunday   Day off

Continuity Clinic:No Continuity Clinic during nights
