Instructions

Students

  1. Complete the Physical Exam Self-Assessment during week one and again in week five.
  2. Enter data into your PDA for ALL patients seen during weeks one and two of your rotations in BOTH family medicine and ambulatory medicine. This means you will repeat this process in weeks five and six as you move into your second placement in this clerkship.
  3. Run an aggregate report on your patient data at the conclusion of week two (or week six) to review prior to meeting with your preceptor/site director.
  4. Approach your preceptor/site director for a feedback session to discuss your experiences at this site, and ensure that you are observed or will be observed in the Mini CEX format. Please bring your completed physical exam self-assessment form and patient data report from that clerkship to your preceptor to discuss your progress.
  5. Agree upon a time for the Mini CEX.
  6. Return both forms (from FM & IM) on the final Friday of clerkship prior to taking your exam.

Faculty (when meeting with student)

  1. Please review and discuss this student's patient data, educational goals and Physical Exam Self-Assessment Form near the midpoint of their clerkship (as they conclude the second or sixth week). The student will provide you with these items for your review. If there are conditions/diseases that the student has not yet encountered please attempt to ensure that these will be seen prior to the conclusion of their clerkship.
  2. Plan a time for completion of the Mini CEX.
  3. Please sign the Mini CEX form stating that the student has completed data collection, been given formal feedback and been observed.
